$150 if either you or your covered spouse/same-sex domestic partner complete both steps
$300 if both you and your covered spouse/same-sex domestic partner complete both steps

Step 1
Get a health screening to identify your numbers for:

Blood pressure
Cholesterol
Body mass index
Blood glucose

Click here to download/print a form for your provider to complete then:
- You are encouraged to schedule your annual preventive visit with your PCP to obtain your screening numbers.
- You can go to a UH Giant Eagle Fast Care Clinic or make an appointment with Corporate Health at Case Medical Center for screenings.
If you have had a screening since October 1, 2010, your information will be transferred to the new Health Assessment with Apex WebMD.

Step 2
Complete the confidential online health assessment and get your results and recommended action steps for improving your health.

Beginning January 1, 2012, complete the new Health Quest Survey through Apex WebMD. Look for your introductory letter from Apex WebMD mailed to your home, which will contain your personal identification number (PIN). You will also need your member ID number to register to take the Health Quest Survey.
