MetLife Critical Illness Insurance is designed to work with your medical and disability coverage to provide financial protection from costs associated with having a critical illness. It can help you pay your deductible and other medical costs or general living expenses like mortgage payments and child care – you have the flexibility to use the benefit in any way you choose. Because Critical Illness Insurance supplements and does not replace medical insurance, you must have medical insurance to enroll in this coverage (although it is not required that your coverage be through UH). Here is an overview of this new benefit option.

How It Works

Critical Illness Insurance provides you with a lump-sum benefit of $15,000 in the event you or your covered dependent experience one of the following medical conditions:

  • Cancer
  • Heart attack
  • Heart transplant
  • Stroke
  • Major organ transplant
  • Kidney failure

View the Critical Illness Insurance Disclosure Document for more information on how coverage works. This Critical Illness Insurance is not considered a UH-sponsored ERISA benefit plan. UH is simply making this coverage option available to UH employees. All the terms and conditions of such coverage are described in the MetLife materials made available to you.

Eligibility

Any regular full- or part-time employee who is actively at work and working 16 or more hours per week (or an FTE status of .20 or more for UHMG physicians) is eligible for coverage. Your spouse or same-sex domestic partner and dependent children up to age 26 are also eligible.

Coverage Options

You can choose coverage for:

  • Yourself – $15,000 in coverage
  • You and your spouse/same-sex domestic partner – $15,000 each
  • You and your dependent child(ren) – $15,000 for you and $10,000 for each eligible dependent child
  • You and your family – $15,000 each for you and your spouse/same-sex domestic partner and $10,000 for each eligible dependent child

Note: In the case of coronary artery bypass and certain types of cancer, the insurance pays a benefit of 25% of the full amount – and the remaining 75% is payable if you experience another covered condition in the same category.

No evidence of insurability is required if enrolling during annual enrollment. There is a waiting period before you become eligible for benefits, and pre-existing condition rules may apply.
