You and your covered spouse/same-sex domestic partner are eligible to earn wellness credits if enrolled in a UH medical plan. You can reduce your medical coverage costs by up to $600 for individual coverage or up to $1,200 if you cover your spouse or same-sex domestic partner. Getting Started: Your Member Number

To get started immediately on the wellness activities that can be completed during open enrollment, click the Quick Link to Apex Plan Central. You will receive a welcome letter from Apex with your registration information. The Apex member number for you or your spouse/same-sex domestic partner consists of UH + the last 7 digits of your UH employee number + 00 for employee or 01 for spouse/same-sex domestic partner. Your UH employee number is listed in your Employee Direct Access record.

There is still time to complete the required steps. You will receive semi-monthly credits in 2012 only for the time following your completion.

Identify a PCP and agree to participate in a disease or case management program if eligible
  • Earn up to $150 if you or your covered spouse/same-sex domestic partner complete this step
  • Earn up to $300 if both you and your covered spouse/same-sex domestic partner complete this step

Complete a tobacco attestation

  • Earn up to $150 if you or your covered spouse/same-sex domestic partner complete this step
  • Earn up to $300 if both you and your covered spouse/same-sex domestic partner complete this step

If you use tobacco, you are still eligible for the wellness credits if you participate in a tobacco cessation program.

Complete these steps by 5/31/2012 to earn full wellness credits:

Get a health screening and take the online health assessment

  • Earn up to $150 if you or your covered spouse/same-sex domestic partner complete both steps
  • Earn up to $300 if both you and your covered spouse/same-sex domestic partner complete both steps

If you’ve completed a health screening since 10/1/2010, you can use those screening numbers to complete your 2012 health assessment.

Complete this step by 6/30/2012 to earn full wellness credits:

Participate in a wellness program offered by UH

  • Earn up to $150 if you or your covered spouse/same-sex domestic partner complete a program
  • Earn up to $300 if both you and your covered spouse/same-sex domestic partner complete a program

Any wellness credits you earn will be divided up and paid throughout the year. Each wellness credit opportunity is equivalent to a $6.25 credit paid via payroll.

If you complete required steps for the credits with a May 31st or June 30th deadline by their end date, you can also receive credits for the period between January 1, 2012 and the deadline – and semi-monthly credits for the rest of the year. If you miss the deadline but still complete the required steps, you will receive semi-monthly credits in 2012 only for the time following your completion.
