Both plans are self-funded by University Hospitals, which means that University Hospitals pays the plan’s portion of all medical and pharmacy claims. Beginning 1/1/2012, Apex Benefits Services will provide administrative services, including member services and claims processing. See who’s eligible for UH medical coverage.
The provider and facility network used to support both medical plans offered by University Hospitals has been adjusted to optimize services to all covered members. Please review the sections below for information on utilizing the medical plans and the supporting network.
How the Plans Work
Both University Hospitals employee medical plans pay benefits based on the provider or facility you select. Both plans also cover preventive care at 100% in-network and have an unlimited lifetime maximum benefit. |

University Hospitals Accountable Care Organization (UHACO)
A key foundation of the UHACO is to utilize a selected set of providers and facilities to enhance the patient experience for all UHACO members by delivering care aligned with best practice clinical protocols and quality standards, coordinating care across multiple providers, and helping members access appropriate wellness programs.
The UHACO Provider Network is comprised of over 1,300 UH employed physicians and over 1,400 independent physicians and providers. The provider network will be supported by the UHACO Facility Network, which includes all University Hospitals inpatient facilities and outpatient treatment centers, two University Hospitals joint venture inpatient facilities, and selected University Hospitals partnerships.

New Medical Plan Administrator
Beginning January 1, 2012, Apex Benefits Services will replace Anthem Blue Cross and Blue Shield as the administrator for the UH medical plan.
If you enroll in medical coverage for 2012, you will receive new ID cards from Apex Benefits Services to use for care you receive in 2012. If you do not receive ID cards by 1/1/2012, contact Apex Benefits Services at 1-800-753-8429.
Due to the transition to Apex, you will receive a required HIPAA notice called a Certificate of Coverage from Anthem with a coverage termination date of 12/31/ 2011. However, your coverage will then begin with Apex effective 1/1/2012. To verify your coverage, you may enter EDA at any time to review your benefits. After 1/1/2012, go to Apex Plan Central to view your coverage. |
