Each medical plan option has a different benefit structure for deductibles, copays, coinsurance amounts, out-of-pocket maximums, and employee contributions depending on the provider or facility you choose. Here is an overview.

Care from Tier 1 (UHACO) or Tier 2

You will receive the highest level of benefit if you choose a Tier 1 (UHACO) provider. Through your benefit structure, you also have access to Tier 2 providers.

 

If you go to providers or facilities that are not in network, the plan pays the lowest level of benefits (Tier 3).

Locating a Provider or Facility inside Ohio

Click here to find a UHACO facility. You can search for a Tier 1 (UHACO) or Tier 2 provider by location using the Apex Provider Directory Search.

Visit the Apex Provider Directory Search and follow these steps:

  • Enter your search criteria. You may search by provider name, specialty or location
  • Narrow your search by entering additional criteria, if desired, including language spoken, gender and extended office hours, then click “continue.”
  • Select “My plan’s network” and enter the group number on your Member ID card and click “search.” If you have not yet received your Member ID card, use the following temporary group number below: G02595000

Locating a Provider or Facility outside of Ohio

You can search for a Tier 2 provider by using the PHCS provider directory.

  • Click here to visit the PHCS website.
  • Under the “Front of Card” listing, select the PHCS logo.
  • Enter your search criteria.

All providers listed are covered at the Tier 2 benefit level.

Travel Benefits

If member lives outside of the UHACO Service Area but within Ohio:

  • If you are traveling inside Ohio: visit the Apex Provider Directory Search and select “My plan’s network” and enter the group number on your Member ID card (or temporary group number G02595000).
  • If you are traveling outside of Ohio, use MultiPlan (under back of card, select MultiPlan).

If member lives outside of the UHACO Service Area and outside Ohio:

  • If you are traveling into Ohio: visit the Apex Provider Directory Search and select “My plan’s network” and enter the group number on your Member ID card (or temporary group number G02595000).
  • If you are traveling anywhere outside of Ohio: visit the PHCS search and under the “Front of Card” listing, select the PHCS logo. You can find additional providers utilizing MultiPlan (under “Back of Card” select MultiPlan).
* In case of an emergency situation, Tier 1 benefits will apply regardless of provider.
