As part of enrolling in the UH medical plans, UH employees and their dependents will receive disease or case management services as their medical claims, medical claims history, medication usage, or health assessment may indicate is appropriate. These services will help you manage your care more effectively and assist you in accessing available resources. Your health information will be kept confidential, and you are able to designate your preferred contact information for the disease or case management services through Employee Direct Access (EDA) in Oracle.

Please note that your agreement to participate in disease or case management if eligible indicates that you will engage with Apex Benefits Services and/or the UHACO if you are contacted. If you do not agree to participate in disease management or case management if eligible, information related to your medical claims, medical claims history, medication usage, or health risk assessment will still be shared for the purposes of disease management and case management and you may still be contacted by Apex Benefits Services or the UHACO. Your health information will be kept confidential and you are able to designate your preferred contact information for the disease management and case management services through EDA.

  • Take care of yourself and your newborn after delivery and more!

If you’re enrolled in a UH medical plan, you can earn wellness credits that help offset your cost of medical coverage – your covered spouse/same-sex domestic partner can too!
