| You are eligible to enroll
in the UH Benefits Program if: |
- UH physicians: you have an FTE status of .20 or higher
- All other employees: you are a regular status employee scheduled to work 16 or more hours a week
As an eligible employee, you can elect to participate in the benefits described on this site after you complete the waiting period specified for each benefit, unless otherwise noted. |
| You may cover eligible dependents, including: |
- Your legal spouse or same-sex domestic partner if qualifications are met (note: employer-paid portion of same-sex domestic partner coverage is taxable to employees)
- Your unmarried dependent child(ren) until the end of the year in which he or she turns age 19 (or until the end of the year in which he or she turns age 25 if a full-time student at an accredited college or university) and who qualifies as a tax dependent for you, your spouse or same-sex domestic partner, including:
- Natural and legally adopted children, children placed with you for adoption, or any other children for whom you or your spouse is named legal guardian, according to a letter of guardianship
- Stepchildren or your same-sex domestic partners’ children who are dependent on your spouse for support
- Biological or legally adopted children for whom the plan is obligated under a Qualified Medical Child Support Order (QMCSO) to provide medical coverage.
- Your unmarried child who became disabled prior to age 19 (or age 25 if a full-time student) and is incapable of self-support due to mental retardation or physical or mental handicap and is primarily dependent on you for support and maintenance.
Documentation will be required to prove dependent and/or disability status. |
