Introducing Apex WebMD Health Quest Survey
To receive your 2012 wellness credits, you must complete the new online health assessment through Apex WebMD. Click here for instructions on how to register. For your convenience we have also added Apex WebMD to Quick Links.
Look for your introductory letter from Apex WebMD mailed to your home, which will contain your personal identification number (PIN). You will also need your member ID number to register to take the Health Quest Survey. Your personal data has been preloaded if you have completed a health screening after October 1, 2010. This data is protected and you will need your PIN in order to view your health screening numbers. Please note that the only results that will be viewable after you enter the PIN are results through a previous UH on-site health screening or results faxed to the UHACO by your PCP, Corporate Health or a UH FastCare Clinic.
Know the Connection
If any of these numbers are above “healthy targets,” you increase your risk of developing a serious health problem. Consider these facts:
- High blood pressure is the #1 cause of stroke.
- There is a two times greater risk of heart attack if you have high cholesterol.
- The risk of arthritis in the knee is 4 to 5 times greater if you are obese.
- Diabetes causes 44% of new cases of kidney failure. By knowing your numbers, you’re taking a first step toward improving them.

Have you completed a health screening since 10/1/2010?
You can use those screening numbers in the 2012 online health assessment. Your health numbers will transfer automatically from your current health assessment to the new Apex WebMD health assessment. To receive wellness credits, you must complete the WebMD assessment.
Do you need your health numbers?
Click here to download a form to complete at your next PCP appointment, at a UH Giant Eagle Fast Care Clinic or at an appointment with Corporate Health at Case Medical Center. Look for more information about on-site screening dates in January.
Remember the March 31st deadline: Be sure to complete the health screening and online health assessment by 3/31/2012 to earn full credit for 2012. If you complete after the deadline, you will receive semi-monthly credits in 2012 only for the time following your completion. |
