Important!
If you do not designate a beneficiary for life insurance, your benefits will be paid upon your death to the first of the following:

  • Your surviving spouse
  • Your surviving child(ren)
  • Your surviving parents
  • Your surviving siblings
  • Your estate

Designate Your Life Insurance Beneficiary Online

To designate your beneficiary for life insurance, go to Prudential’s Beneficiary Designation web site any day, anytime. Follow these steps to name or re-designate your beneficiary:

1. Click here to access the Beneficiary Designation website (Note: the site is not compatible with MAC or Firefox.)
Register First:
2. Select "Register Here” under First Time User 
3. Enter “51250” in the Control Number field
4. Complete the required information and create your personal User ID and personal password
5. Log In and Designate your beneficiary − Enter your User ID and Password
6. Click on View/Change Beneficiary
7. Complete the requested information and click Save to List
8. Complete the Share %
9. Be sure to click on SUBMIT when your designation is completed

You will receive a beneficiary confirmation e-mail with your designation to the e-mail address you provided upon registration.

Use Fidelity NetBenefits® to name or re-designate your beneficiaries online. It’s easy and convenient and takes just minutes:

  • Go to NetBenefits
  • Click Your Profile, at the top of the page
  • Click Beneficiaries in the About You section.
